ACFI and Documentation Co-ordinator- Bundaberg

Recruitment 24/7 has an opportunity with one of their major clients for an experienced ACFI and Documentation Co-ordinator (working in Aged Care residences) in Bundaberg.

In this full time role, you will undertake assessments and appraisal of resident care needs using the Aged Care Funding Instrument (ACFI) and submit ACFI claims.  You will secure optimal funding for the benefit of residents and the facilities.  This role is ideally suited to an RN with 3 to 5 years’ experience in Aged Care, who is committed to achieving great care for residents and has excellent inter-personal and communication skills.

Duties for the role:

  • Conduct ACFI assessments, appraisals and reviews, working collaboratively with Senior Management at the facility, GPs, Allied Health Professionals and direct care staff
  • Optimise ACFI funding
  • Review ACFI documentation, ensuring all assessments and submissions are accurate and up-to-date
  • Enter ACFI data and monitor appraisal and re-appraisal expiry dates, annual reviews and new admissions
  • Review ACFI processes to achieve the best outcomes
  • Maintain contemporary ACFI knowledge
  • Share ACFI knowledge and information through the delivery of training and education to staff.

The following criteria must be met:

  • Proven experience in achieving outcomes through assessment and documentation coordination
  • Experience using ACFI and knowledge of the ACFI framework
  • Ability to assist and provide information to authorities during ACFI validations
  • High level of planning, organisation and prioritisation skills
  • Ability to identify training needs and experience in delivering ACFI and documentation training
  • Advanced Knowledge of Microsoft Office and the Aged Care Online system
  • Experience using a computerised Care Management System
