Recruitment 24/7 has currently has a vacancy for a Chronic Disease Support Worker working with one of our clients in the Dalby Region. This is a permanent role with a very attractive salary package on offer for the right person position.
You will be supporting the delivery of Chronic Disease Services (CDS) in accordance with the current Chronic Disease Services Plan and other Service Plans as required. Other duties also include liaising with other team members, Coordinators, the Clinic Manager and the Service Planning Manager to ensure the Health Service delivers effective and efficient holistic primary health care to chronic disease patients.

The role of the Chronic Disease Support Worker is wide and varied. I can Guarantee the successful applicant will not get bored in this role. You will be working alongside a group of Doctors within multiple practices along with an experienced Chronic Disease Nursing team.
Qualifications and skills required:
Current Registered Nurse (RN) / Endorsed Enrolled Nurse (EEN) / Enrolled Nurse (EN) Registration through the Australian Health Practitioners Regulation Agency would be advantageous
Other relevant Health Qualifications i.e. Certificate/Diploma in Primary Health Care
Be in possession or be eligible for Working with Children Blue Card (Queensland)
Possess a current Queensland Driver’s License
Be in possession or be willing to obtain First Aid and/or CPR Certification
The start for this position is ASAP. The successful applicant must be able to pass a drug test before commencement of your assignment.
